JOB TITLE:  Prevention Case Manager

The Latin American Health Institute seeks candidates for a full -time prevention and screening counselor to provide HIV services to Latino gay men at high risk for HIV and to HIV+ Latinos in need of education and support.

Reports to: Prevention & Screening Program Coordinator

Responsibilities include:

• Conduct targeted outreach to the priority population. This may include some evenings and weekends.
• Screen program participants for eligibility of services
• Complete risk assessment, risk reduction plans to all eligible participants
• Meet with individuals one-to-one to assist them with completing their risk reduction objectives
• Provide rapid HIV testing to eligible participants
• Develop appropriate advertising materials for the program
• Schedule appointments with program participants to develop risk assessments, risk reduction sessions, provide referrals to services, and provide follow-up for contacted services
• Identify other referrals to address the needs of clients
• Maintain a yearly caseload of 150 high-risk individuals as determined by the risk assessment.
• Keep all required client documentation up to date
• Follow the yearly work plan for the program
• Complete monthly and quarterly service reports as required by LHI and the funding agency
• Attend trainings and group meetings as required by contract
• Attend weekly supervision meetings with program coordinator and clinical supervisor
• Attend LHI unit and staff meetings
• Assist program coordinator in completing other related tasks as requested by supervisor


Qualifications:
Bilingual in Spanish and English. Additional languages preferred but not required. Bicultural preferred. Knowledge and/or experience in working with Latino MSM and Gay communities, SA, HIV/AIDS education and case management. BA or BS or degree and or certificate in Human Health Services or 3 to 5 years prevention and education experience. Good communication, writing and translation skills. Computer literacy required
Must have access to own transportation. Ability to provide presentations in community settings

JOB TITLE:  Home-based Clinician

REPORTS TO: Behavioral Health Unit Director

GOAL: To provide in-home and community-based counseling services to predominantly Latino children with severe emotional disorders, and families struggling with issues of child neglect or abuse.

DUTIES:
• Provide home-based mental health counseling and support services as required.
• Complete Behavioral Health Assessments, outcome scales (e.g.: CANS), screening and other required
documentation as needed including monthly and/or quarterly progress reports.
• Develop therapeutic treatment plans for the duration of the intervention.
• Provide direct clinical interventions to individuals and families.
• Provide therapeutic community- or school-based sessions when required.
• Provide clients with assisted referrals to social service agencies and other supports as outlined by service plan.
• Attend clinical case reviews and providers meetings.
• Maintain good communications with service providers and family members.
• Maintain client records in compliance with program, LHI and State regulations.
• Follow protocols as outlined by the project proposal and/or contract.
• Participate in weekly programmatic supervision
• Participate in weekly clinical supervision.
• Attend Unit and Staff meetings and other administrative meetings as determined by program director.
• Perform other duties relevant to the goals and purposes of the program and the agency as needed.

QUALIFICATIONS:
Applicants must have a Masters degree or equivalent in Counseling, Social Work, Psychology or related field. Bicultural and Licensed (LCSW/LICSW/LMFT/LMHC) candidates are preferred. Training and demonstrated experience in family therapy, and child and adolescent behavioral problems is essential, particularly in treating children with severe emotional disorders and survivors of neglect and physical and sexual abuse. Familiarity with family stabilization models and the wrap around treatment approach is preferred. Applicants should have the ability to work independently and as a member of a multi-disciplinary team, demonstrate sensitivity and awareness of the diverse needs of inner city Latino children, youth and families, and display excellent verbal and written communication skills.

DIRECTOR OF CHILD CLINIC (PART-TIME ~ 2O HOURS)                           

The Latin American Health Institute is seeking to fill the position of Director of its Child Behavioral Health Clinic in Boston. The Director has the responsibility of administrative and clinical supervision of the outpatient clinic that includes a SAMHSA-funded grant. The Director also manages all program activities, budgets and supervision of staff and provides direct services. Candidates should posses a Doctoral or a Master’s Degree in the fields of Psychology, Social Work, Counseling Psychology or Psychiatric Nursing, a current and valid independent license, and 5 or more years of management experience. Bilingual in Spanish and English is preferred.

CHILD CLINICIAN (FULL-TIME)                                                                       

The Latin American Health Institute is seeking to fill the position of Child Clinician in its Child Behavioral Health Clinic in Boston. The Child Clinician has the responsibility of providing trauma-focused clinical services to Latino children and youth and their caregivers who have faced traumatic experiences. Clinician will conduct trauma screenings, assessments, and evaluations and develop and implement treatment plans. Clinician will also facilitate treatment groups, parenting groups, and other psycho-educational groups as needed. Candidates should posses a Master’s Degree in the fields of Social Work or Counseling Psychology and preferably be licensed. Must be bilingual in Spanish and English.  Knowledge and experience using CBT/DBT approaches as well as knowledge/training in childhood trauma is also preferred.

CLINICAL CASE MANAGER (FULL-TIME)                                                     

The Latin American Health Institute is seeking to fill the position of Clinical Case Manager in its Behavioral Health Program in Boston. The Clinical Case Manager will provide Community Services Program (CSP) and Therapeutic Mentoring (TM) services to children, adolescents and adults. CSP provides essential services to individuals with a long-standing history of a psychiatric or substance use disorder and to their families, or to individuals who are at varying degrees of increased medical risk, or to children/adolescents who have behavioral health issues challenging their optimal level of functioning in the home/community setting. TM offers structured, one-to-one, strength-based support services between a therapeutic mentor and a youth for the purpose of addressing daily living, social, and communication needs. Candidates should possess a BA/BS in Human Services from an accredited university and one year experience working with children/adolescents/transition age youth. Must be bilingual in Spanish and English. Preference will be given to candidates who can demonstrate knowledge of substance abuse, family therapy, reunification model and adolescent behavior.  Must be willing to work flexible hours and have use of their own car.
