The main goal of the Social Services Unit is to improve the health and well being of individuals and families in the Latino community, through culturally appropriate services, in the areas of housing options, client advocacy, case management services for people living with HIV, and family support services for people with disabilities. The Unit offers the following programs:
Proyecto Busqueda
This bilingual/bicultural, housing search and
placement program assists Latinos living with HIV/AIDS to access,
obtain, sustain and succeed in seeking adequate housing. The program
focuses on the provision of competent client advocacy services. LHI has
provided this service since 1994. Case coordination and collaboration
with other providers have insured that services are not duplicated, and
individuals receive quality services. The program provides intensive
hands-on support to help HIV+ Latinos obtain housing and other benefits.
Horizontes Advocacy & Case Management Services for people living with HIV:
The objective of this program is to link HIV-positive
Latinos with health care and other important programs and resources,
and to provide competent and appropriate interpretation and advocacy
services for families living with HIV/AIDS. The program also offers
client accompaniment, information and referrals to health and social
services, and referrals for substance abuse treatment and mental health
services.
Opciones – Assisted Living Program:
This program is designed to help
low-income families, homeless individuals, or people living with
HIV/AIDS to secure housing by assisting with housing search and by
providing subsidies for low-income residents. The program has a case
management component that assiss individuals to access additional
support and referrals to services. The objective of this program is to
help participants to achieve a higher quality of life and increase
their independance and self-sufficiency.
Solidaridad – Family Support Services:
This program provides services
to Latino families with children with disabilities, by coordination
services and referrals to appropriate natural supports and specialized
services and referrals to appropriate natural supports and specialized
services within the community. Family governance and active involvement
of our participants in identifying their own needs are the guiding
principles of the program.
Walnut Park – Supportive Housing Program:
This is a supportive housing
option for persons living with HIV/AIDS, to help participants who need
additional housing supports and services. The program provides 13
studios and one-bedroom apartments within the Walnut Park community.
The goals of the program are to provide an environment that is
conductive to growth and development, and help participants to achieve
residential stability, increase skills and income and achieve greater
self-determination.
Adolescent Network for Today (ANT):
The ANT program serves HIV-
positive youth between the ages of 13 and 24 from all racial, ethnic
and economic backgrounds. ANT provides supportive services to help HIV
- positive youth to stay in care, and links youth who are not in care
to medical services and other supports.
